The form below must be submitted prior to every training session, match or club event – but NO MORE THAN 24 HOURS IN ADVANCE – one form is required per player or volunteer.

IF YOU ARE UNABLE TO CONFIRM YOU ARE COVID AND SYMPTOM FREE, OR AWAITING THE OUTCOME OF A PCR TEST FOLLOWING POSSIBLE ‘CLOSE CONTACT’, PLEASE DO NOT ATTEND.
